Key Responsibilities & Duties
- Perform home care assessments to determine patient eligibility and develop comprehensive care plans.
- Provide skilled nursing care, including wound care, injections, and medication management.
- Supervise and orient home health aide personnel, ensuring compliance with regulatory standards.
- Coordinate care with other disciplines such as physical therapy, speech therapy, and nutrition.
- Monitor patient progress and consult with physicians to adjust treatment plans as needed.
- Complete required documentation, including 485 forms and medication profiles, ensuring accuracy and timeliness.
- Participate in case conferences and communicate effectively with patients and families.
- Identify and address changes in patient clinical, psychological, and functional status.
Job Requirements
- Associate of Applied Science (AAS) in Nursing from an accredited institution.
- Current Registered Nurse (RN) license in New York State.
- Minimum of 1 year of experience in home care or CHHA settings.
- Proficiency in care coordination and multidisciplinary team collaboration.
- Strong verbal and written communication skills for effective patient and team interactions.
- Ability to complete comprehensive assessments and develop patient-centered care plans.
- Knowledge of healthcare documentation standards and compliance requirements.
