Key Responsibilities & Duties

  • Perform home care assessments to determine patient eligibility and develop comprehensive care plans.
  • Provide skilled nursing care, including wound care, injections, and medication management.
  • Supervise and orient home health aide personnel, ensuring compliance with regulatory standards.
  • Coordinate care with other disciplines such as physical therapy, speech therapy, and nutrition.
  • Monitor patient progress and consult with physicians to adjust treatment plans as needed.
  • Complete required documentation, including 485 forms and medication profiles, ensuring accuracy and timeliness.
  • Participate in case conferences and communicate effectively with patients and families.
  • Identify and address changes in patient clinical, psychological, and functional status.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing from an accredited institution.
  • Current Registered Nurse (RN) license in New York State.
  • Minimum of 1 year of experience in home care or CHHA settings.
  • Proficiency in care coordination and multidisciplinary team collaboration.
  • Strong verbal and written communication skills for effective patient and team interactions.
  • Ability to complete comprehensive assessments and develop patient-centered care plans.
  • Knowledge of healthcare documentation standards and compliance requirements.
