Overview
- Join a dedicated healthcare organization as a Registered Nurse, providing essential home care services to patients in the community.

- Conduct comprehensive assessments and implement personalized care plans in collaboration with patients, families, and physicians.
- Provide skilled nursing care, including wound care, medication management, and disease education.
- Supervise and orient home health aides, ensuring compliance with regulatory standards.
- Evaluate care effectiveness and adapt interventions to meet evolving patient needs.
- Collaborate with interdisciplinary teams for comprehensive patient support and care.
- Work onsite, Monday to Friday, with no weekend shifts required.
Key Responsibilities & Duties
- Perform home care assessments to determine patient eligibility for services and document findings.
- Develop and implement personalized care plans based on comprehensive assessments.
- Provide skilled nursing care, including wound care, injections, and medication management.
- Supervise and orient home health aide personnel, ensuring adherence to care standards.
- Coordinate with interdisciplinary teams for holistic patient care and support.
- Monitor patient progress and consult with physicians to adapt treatment plans.
- Complete required documentation, including interim orders and medication profiles.
- Participate in case conferences and address follow-up items promptly.
Job Requirements
- Associate of Applied Science (AAS) in Nursing from an accredited institution.
- Current Registered Nurse (RN) license in New York State.
- Minimum of 1 year of experience in home care or related field.
- Proficiency in conducting assessments and developing care plans.
- Strong verbal and written communication skills for effective coordination.
- Experience with electronic health record systems, particularly HCHB, preferred.
- Ability to work collaboratively with interdisciplinary teams and adapt to patient needs.
