Overview

Key Responsibilities & Duties
- Perform diagnostic and therapeutic procedures within the scope of practice, consulting supervising physicians as needed.
- Provide backup call support for clinical escalations during overnight, holiday, and weekend shifts.
- Ensure accurate documentation and utilization of electronic medical records (EMR) and practice management systems.
- Collaborate with clinical teams to deliver comprehensive care and close gaps in patient treatment.
- Promote a patient-centric model of care to enhance satisfaction and health outcomes.
- Participate in licensing processes to maintain multi-state coverage for on-call responsibilities.
- Engage with community leaders and external stakeholders to foster relationships and improve healthcare delivery.
- Adhere to company policies and procedures to ensure compliance and operational efficiency.
Job Requirements
- Master of Science in Nursing from an accredited institution; active Nurse Practitioner licensure in New York.
- Minimum of 1 year of practice experience, with 3 years preferred; DEA certification required.
- Proficiency in EMR systems and practice management tools; strong communication and presentation skills.
- Ability to travel frequently for patient visits; valid driver’s license and clean record required.
- Commitment to value-based care principles and fostering a culture of empathy and integrity.
- Willingness to obtain and maintain multi-state licensure for on-call support; licensing costs covered by the company.
- Experience in geriatric care and home-based healthcare delivery preferred.
