Key Responsibilities & Duties

  • Manage inpatient caseloads exceeding 35 patients, ensuring quality care and efficient discharge planning.
  • Coordinate with healthcare teams to develop and implement patient care plans.
  • Monitor patient progress and adjust care plans as necessary to meet individual needs.
  • Provide case management services in a 500+ bed hospital setting.
  • Ensure compliance with healthcare regulations and organizational policies.
  • Maintain accurate documentation and reporting for all case management activities.
  • Demonstrate confidence and professionalism in interactions with patients, families, and healthcare staff.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or related field required.
  • Minimum of 2 years of experience in case management or inpatient care.
  • Proven ability to manage caseloads exceeding 35 patients effectively.
  • Experience working in a hospital with 500+ beds preferred.
  • Strong interpersonal and communication skills, demonstrating confidence and professionalism.
  • Ability to work on-site in a high-pressure healthcare environment.
