Job Description

Overview

  • Join a dynamic healthcare team as an LMSW Case Manager on a contract basis.
  • Provide essential social work services in a Level 2 trauma and transplant facility.
  • Collaborate with multidisciplinary teams to ensure effective patient throughput and care coordination.
  • Utilize EPIC and Aiden systems for efficient case management and documentation.
  • Contribute to the care of a diverse patient population with an average daily census of 500.
  • Work on-site in a state-of-the-art medical complex in downtown Houston.

Key Responsibilities & Duties

  • Conduct comprehensive assessments to determine patient needs and develop care plans.
  • Coordinate with healthcare providers to ensure seamless patient transitions and discharge planning.
  • Utilize EPIC and Aiden systems for accurate case documentation and reporting.
  • Advocate for patient rights and access to necessary resources and services.
  • Provide emotional support and counseling to patients and their families.
  • Collaborate with social work and medical teams to optimize patient care outcomes.
  • Participate in team meetings and contribute to case discussions and planning.

Job Requirements

  • Master of Social Work degree from an accredited institution.
  • Active Texas LMSW licensure is mandatory.
  • Minimum of 2 years of professional experience in social work or case management.
  • Proficiency in EPIC and Aiden systems for case management.
  • Strong understanding of patient throughput processes and healthcare coordination.
  • Excellent communication and interpersonal skills for patient and team interactions.
  • Ability to work on-site in a fast-paced medical complex environment.
