Job Description

Overview

  • Provide comprehensive case management services to patients, ensuring optimal care coordination and resource utilization.
  • Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.
  • Monitor patient progress and adjust care plans to meet evolving healthcare needs.
  • Educate patients and families on healthcare options and available resources.
  • Ensure compliance with healthcare regulations and organizational policies.
  • Maintain accurate and up-to-date patient records and documentation.
  • Support transitions of care and discharge planning to promote patient recovery.
  • Participate in quality improvement initiatives to enhance patient outcomes.

Key Responsibilities & Duties

  • Conduct comprehensive assessments to identify patient needs and develop care plans.
  • Coordinate with healthcare providers to ensure seamless delivery of services.
  • Facilitate communication between patients, families, and medical teams.
  • Advocate for patient rights and access to appropriate care resources.
  • Track and report patient outcomes to evaluate care effectiveness.
  • Provide guidance on insurance coverage and financial assistance programs.
  • Participate in team meetings to discuss patient progress and care strategies.
  • Ensure adherence to ethical standards and confidentiality requirements.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent qualification.
  • Minimum of 2 years of experience in case management or related nursing roles.
  • Current RN licensure and certification in good standing.
  • Proficiency in healthcare documentation and electronic medical records systems.
  • Strong knowledge of healthcare regulations and compliance standards.
  • Excellent communication and interpersonal skills for patient interaction.
  • Ability to work effectively in a fast-paced, on-site healthcare environment.
  • Experience with Dignity/CommonSpirit systems within the last 6 months preferred.
