Overview
- Provide comprehensive case management services to patients, ensuring optimal care coordination and resource utilization.
- Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.
- Monitor patient progress and adjust care plans to meet evolving healthcare needs.
- Educate patients and families on healthcare options and available resources.
- Ensure compliance with healthcare regulations and organizational policies.
- Maintain accurate and up-to-date patient records and documentation.
- Support transitions of care and discharge planning to promote patient recovery.
- Participate in quality improvement initiatives to enhance patient outcomes.
Key Responsibilities & Duties
- Conduct comprehensive assessments to identify patient needs and develop care plans.
- Coordinate with healthcare providers to ensure seamless delivery of services.
- Facilitate communication between patients, families, and medical teams.
- Advocate for patient rights and access to appropriate care resources.
- Track and report patient outcomes to evaluate care effectiveness.

Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent qualification.
- Minimum of 2 years of experience in case management or related nursing roles.
- Current RN licensure and certification in good standing.
- Proficiency in healthcare documentation and electronic medical records systems.
- Strong knowledge of healthcare regulations and compliance standards.
- Excellent communication and interpersonal skills for patient interaction.
- Ability to work effectively in a fast-paced, on-site healthcare environment.
- Experience with Dignity/CommonSpirit systems within the last 6 months preferred.
