Overview
- Lead and mentor a team of hospice nurses to deliver exceptional patient care and professional development.
- Support the Hospice Nurse Manager in ensuring compliance with program philosophy, goals, and policies.
- Provide leadership and guidance to professional staff on a day-to-day basis.
- Carry a reduced caseload to balance patient care with supervisory responsibilities.
- Perform fieldwork within the Bronx territory, ensuring high-quality hospice services.
- Engage in mentoring, supervision, and professional development activities for staff.
- Work collaboratively with the team to enhance patient outcomes and staff growth.
- Participate in both virtual and in-person interviews during the hiring process.
Key Responsibilities & Duties
- Provide direct patient care and case management, ensuring adherence to hospice standards.
- Mentor and supervise nursing staff, fostering professional growth and development.
- Assist in orientation and training of new team members to ensure smooth integration.
- Collaborate with the Hospice Nurse Manager to implement program policies effectively.
- Conduct field visits to oversee patient care and provide support to nursing staff.
- Ensure compliance with New York State RN licensure requirements and hospice certifications.
- Utilize software tools such as Word, Outlook, and Windows for documentation and communication.
- Engage in rotating weekend schedules to maintain consistent patient care coverage.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of two years of nursing experience, with four years preferred.
- New York State RN licensure and National Board Certification in Hospice & Palliative Care preferred.
- Proficiency in Word, Outlook, and Windows for effective documentation and communication.
- Ability to supervise and mentor hospice nursing staff effectively.
- Experience in hospice nursing or management is highly desirable.
- Commitment to providing high-quality patient care and professional staff development.
- Flexibility to work rotating weekends and perform fieldwork in the Bronx territory.
