Overview
- Provide high-quality medical care through in-home and virtual visits, ensuring comprehensive assessments, acute visits, and follow-ups for diverse patient needs.
- Collaborate with clinical teams to create individualized treatment plans, embracing a patient-centric model of care and promoting value-based healthcare practices.
- Utilize advanced telemedicine tools for remote diagnostics and therapeutic procedures, ensuring effective and efficient patient care delivery.
- Autonomously manage patient caseloads, scheduling appointments flexibly based on patient availability and clinical requirements.
- Participate in virtual and shadow training sessions to enhance clinical skills and ensure seamless integration into the healthcare team.
- Support the practice management system and electronic medical record (EMR) for accurate documentation and streamlined operations.
- Provide on-call backup coverage for clinical escalations during overnight, holiday, and weekend hours across multiple states.

Key Responsibilities & Duties

- Perform diagnostic and therapeutic procedures within clinical discipline, consulting supervising physicians as necessary.

- Maintain proficiency in practice management systems and electronic medical records for accurate documentation.

Job Requirements
- Master of Science in Nursing from an accredited institution; active unrestricted medical license in New York.
- Minimum of 1 year of experience, with 3 years preferred; DEA certification required or willingness to obtain.
- Proficiency in telemedicine technologies and electronic medical record systems for effective patient care.
- Strong communication skills for clear articulation of concepts and effective collaboration with teams.
- Ability to work autonomously and manage patient caseloads effectively, ensuring high-quality care delivery.
- Experience in home care, hospital, or nursing home settings preferred; comfortable with travel up to an hour.
