Key Responsibilities & Duties

• Conduct home care visits to provide personalized medical attention to patients.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient health and document observations for healthcare teams.
• Educate patients and families about health management and care procedures.
• Ensure compliance with healthcare regulations and standards during visits.
• Maintain accurate and timely patient records for effective communication.
• Utilize your own transportation to travel to patient locations within Westchester County.
• Collaborate with interdisciplinary teams to optimize patient outcomes.

Job Requirements

• Valid Licensed Practical Nurse (LPN) certification is required.
• Minimum of 1 year of experience in a healthcare setting, preferably in geriatrics.
• Associate of Applied Science (AAS) in Nursing or equivalent educational background.
• Reliable personal transportation for fieldwork visits.
• Strong understanding of geriatric care principles and practices.
• Ability to work independently and manage time effectively during field visits.
• Proficiency in documenting patient information and communicating with healthcare teams.
