Job Description
Overview
- Provide comprehensive nursing assessments to medically vulnerable individuals entering the shelter system.
- Collaborate with a multidisciplinary team to determine appropriate shelter placements.
- Contribute to a dynamic environment focused on client health and psychosocial needs.
- Work in a structured schedule, including weekends, to support client outcomes.
- Engage in health education and advocacy to empower clients in achieving their health goals.
- Participate in treatment team meetings and case conferences to optimize care plans.
- Maintain accurate documentation and medication records for assigned clients.
- Provide emergency first aid and CPR assistance as required.
Key Responsibilities & Duties
- Supervise assigned staff and coordinate client medical and psychosocial needs.
- Act as primary medical case manager, conducting health checks and coordinating care.
- Collaborate with physicians, pharmacies, and other treatment providers for optimal client outcomes.
- Document findings and maintain medication administration records and controlled substance logs.
- Provide health education and advocate for client health preferences and goals.
- Participate in team meetings and case conferences to share insights and recommendations.
- Ensure adequate medication supply and follow up on client needs.
- Deliver emergency first aid and CPR assistance when necessary.
Job Requirements
- Valid Licensed Practical Nurse (LPN) license required.
- Associate of Applied Science (AAS) degree in nursing or related field.
- Minimum of 1 year of nursing experience; 2 years preferred.
- Experience in medical, psychiatric, and substance use care is advantageous.
- Strong documentation and record-keeping skills.
- Ability to work collaboratively within a multidisciplinary team.
- Proficiency in health education and advocacy for diverse populations.
