Job Description

Overview

  • Provide essential nursing care in a 32-bed detox and rehabilitation unit.
  • Collaborate with a well-staffed team including experienced RNs and healthcare professionals.
  • Work a consistent Monday-Friday daytime schedule, ensuring work-life balance.
  • Contribute to client health management through direct care and coordination.
  • Engage in health education and advocacy to support client wellness goals.
  • Utilize your LPN expertise to manage medical casework and documentation.
  • Participate in treatment team meetings to optimize client outcomes.

Key Responsibilities & Duties

  • Supervise assigned staff and coordinate client medical and psychosocial needs.
  • Act as primary medical case manager, conducting health checks and coordinating care.
  • Maintain medication administration records and controlled substance logs.
  • Provide health education and advocate for client health preferences.
  • Collaborate with treating physicians, pharmacies, and other healthcare providers.
  • Participate in treatment team meetings and case conferences.
  • Ensure adequate medication supply and follow-up on client health needs.
  • Provide emergency first aid and CPR assistance as required.

Job Requirements

  • Valid LPN license required with a minimum of 1 year of experience.
  • Preferred 3 years of experience in detox or rehabilitation settings.
  • Associate of Science (AS) degree in Nursing or related field.
  • Strong documentation skills for maintaining medical records and logs.
  • Proficiency in coordinating with multidisciplinary healthcare teams.
  • Ability to provide health education and advocate for client wellness.
