Job Description

Overview

  • Provide comprehensive medical case management for clients in permanent supportive housing.
  • Collaborate with healthcare providers to optimize client health outcomes.
  • Conduct health checks and coordinate necessary medical treatments.
  • Assist clients with practical needs and advocate for their preferences.
  • Document client progress and maintain accurate medication records.
  • Work closely with interdisciplinary teams to address psychosocial needs.
  • Provide health education and emergency first aid as required.
  • Support clients in achieving better outcomes through service connections.

Key Responsibilities & Duties

  • Supervise assigned staff and ensure adherence to medical protocols.
  • Link clients to appropriate medical and psychiatric services.
  • Coordinate with physicians, pharmacies, and other treatment providers.
  • Manage client health needs through regular health assessments.
  • Maintain medication administration records and controlled substance logs.
  • Advocate for client health goals and preferences in treatment planning.
  • Provide emergency medical assistance when necessary.
  • Participate in treatment team meetings and case conferences.

Job Requirements

  • Active LPN license required with a minimum of 1 year of experience.
  • Preferred 3 years of experience in medical case management.
  • Associate of Science (AS) degree in Nursing or related field.
  • Proficiency in maintaining accurate medical records and documentation.
  • Strong ability to coordinate with interdisciplinary teams and healthcare providers.
  • Knowledge of psychiatric rehabilitation and medical treatment referrals.
  • Capability to provide health education and emergency first aid.
