Overview
- Provide comprehensive medical case management for clients in permanent supportive housing.
- Collaborate with healthcare providers to optimize client health outcomes.

Key Responsibilities & Duties
- Supervise assigned staff and ensure adherence to medical protocols.
- Link clients to appropriate medical and psychiatric services.
- Coordinate with physicians, pharmacies, and other treatment providers.
- Manage client health needs through regular health assessments.
- Maintain medication administration records and controlled substance logs.
- Advocate for client health goals and preferences in treatment planning.
- Provide emergency medical assistance when necessary.
- Participate in treatment team meetings and case conferences.
Job Requirements
- Active LPN license required with a minimum of 1 year of experience.
- Preferred 3 years of experience in medical case management.
- Associate of Science (AS) degree in Nursing or related field.
- Proficiency in maintaining accurate medical records and documentation.
- Strong ability to coordinate with interdisciplinary teams and healthcare providers.
- Knowledge of psychiatric rehabilitation and medical treatment referrals.
