Overview
- Provide comprehensive nursing care to patients in home settings, ensuring quality and compliance with care plans.

Key Responsibilities & Duties
- Conduct home visits and perform comprehensive assessments using OASIS tools.
- Develop and implement individualized care plans in collaboration with patients and families.
- Provide skilled nursing care including injections, wound care, and medication preparation.
- Supervise and orient home health aides, ensuring adherence to care plans.
- Coordinate interdisciplinary services such as physical therapy and nutrition counseling.
- Monitor patient progress and communicate changes to physicians and care teams.
- Document all care activities and patient responses in clinical software systems.
- Participate in discharge planning and ensure smooth transitions for patients.
- Engage in quality improvement activities and mandatory training sessions.
Job Requirements
- Associate of Applied Science (AAS) in Nursing from an accredited institution.
- Active Registered Nurse (RN) license in the state of New York.
- Minimum of 1 year of hospital experience; CHHA experience preferred.
- Proficiency in using clinical software systems for documentation and care coordination.
- Strong verbal and written communication skills for effective patient and team interactions.
- Fully vaccinated for COVID-19 as per organizational requirements.
- Availability to work 13-hour shifts on Fridays, Saturdays, and Sundays.
- Commitment to infection control standards and quality care delivery.
- Ability to manage caseloads and coordinate interdisciplinary services effectively.
