Job Description

Overview

  • Serve as an RN Admissions Evaluator in a dynamic healthcare environment, ensuring quality assessments and admissions processes.
  • Conduct pre-admission assessments and liaise with hospitals and managed care organizations.
  • Provide thorough clinical and psychosocial evaluations for potential applicants.
  • Collaborate with healthcare professionals to optimize patient admissions and care plans.
  • Utilize electronic medical records and Microsoft Office tools effectively.
  • Contribute to monthly reporting and statistical analysis for admissions processes.
  • Enhance patient and family experiences through facility tours and consultations.
  • Work onsite in a fast-paced environment with adult and geriatric populations.

Key Responsibilities & Duties

  • Review PRI documents and clinical documentation for admissions suitability.
  • Coordinate with hospitals and referral services to identify and evaluate applicants.
  • Complete pre-admission assessments and gather additional medical and social information.
  • Initiate application processes and provide program information to applicants.
  • Document clinical and psychosocial assessments accurately and comprehensively.
  • Obtain pre-authorizations from managed care companies and ensure compliance with level requirements.
  • Conduct facility tours for potential patients and their families.
  • Provide weekend and holiday admissions coverage as required.

Job Requirements

  • Bachelor of Science in Nursing (BSN) and RN registration in New York State.
  • Minimum of 2 years of experience in post-acute admissions processes.
  • Acute care experience and PRI certification preferred.
  • Proficiency in electronic medical records and Microsoft Office tools.
  • Excellent clinical assessment and documentation skills.
  • Ability to multi-task and maintain accuracy in a fast-paced environment.
  • Strong interpersonal skills and ability to work well with diverse populations.
  • Knowledge of medical terminology and treatments for adult and geriatric care.
