Overview
- Provide compassionate social work services to patients and families facing serious illnesses.
- Collaborate with interdisciplinary teams to deliver holistic, patient-centered care.
- Support emotional, social, and practical needs of patients and caregivers.
- Facilitate discussions on advance directives and end-of-life planning.
- Advocate for patient preferences and cultural values in care decisions.
- Conduct home visits, assessing patient needs and providing tailored support.
- Maintain compliance with documentation standards and organizational policies.
- Participate in shared weekend rotations as part of the care team.
Key Responsibilities & Duties
- Conduct comprehensive assessments to identify patient and caregiver needs.
- Provide emotional and psychosocial support to patients and families.
- Assist with navigating healthcare systems and accessing community resources.
- Collaborate with interdisciplinary teams to create personalized care plans.
- Document patient interactions and care plans in electronic medical records.
- Facilitate discussions on care goals and advance planning directives.
- Provide crisis intervention during acute patient or caregiver challenges.
- Participate in team meetings and contribute to holistic care strategies.
Job Requirements
- Master's degree in Social Work (MSW) from an accredited program.
- NYS LMSW license required; NYS LCSW preferred.
- Experience in palliative care, hospice, or medical social work preferred.
- Knowledge of serious illness, grief, and community resources.
- Strong communication and clinical documentation skills.
- Ability to work independently and collaboratively within a team.
- Valid driver’s license and reliable transportation for home visits.
- Commitment to patient-centered care and cultural sensitivity.
