Job Description

Overview

  • Provide physical therapy services in a community-focused adult day care program.
  • Part-time contract role with potential for full-time as the program grows.
  • Work hours are Monday to Friday, 9:00 AM to 2:00 PM.
  • Engage with an aging population in a state-of-the-art facility.
  • Opportunity to contribute to restorative and maintenance therapy programs.
  • Dates requiring coverage include November and December of 2025.
  • Collaborate with a multidisciplinary team to enhance client care.

Key Responsibilities & Duties

  • Perform daily assessments and treatments for clients in the program.
  • Complete all necessary documentation related to client care and progress.
  • Develop and implement individualized therapy plans for clients.
  • Provide restorative and maintenance therapy tailored to client needs.
  • Work collaboratively with other healthcare professionals to optimize outcomes.
  • Ensure compliance with state regulations and professional standards.
  • Adapt therapy approaches to meet the unique needs of an aging population.

Job Requirements

  • Bachelor's degree in Physical Therapy or related field required.
  • Current New York State Physical Therapist license mandatory.
  • Minimum of 2 years of experience as a Physical Therapist.
  • Preferred 5 years of experience working with aging populations in community settings.
  • Proficiency in restorative and maintenance therapy techniques.
  • Bilingual Spanish-speaking candidates preferred.
  • Strong documentation and communication skills essential.
