Overview
- Provide compassionate care to patients with chronic or life-limiting illnesses in a community-based setting.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans.
- Conduct home visits and assessments to manage symptoms and address patient needs.
- Support patients and families with emotional and psychosocial guidance.
- Participate in program development for home-based palliative care services.
- Ensure accurate clinical documentation and compliance with regulatory standards.
- Diagnose and treat acute, episodic, or chronic illnesses effectively.
- Prescribe necessary medications and treatments to manage patient conditions.
- Engage in community and professional education initiatives related to hospice care.
Key Responsibilities & Duties
- Conduct in-depth palliative care assessments and manage patient care plans collaboratively.
- Perform face-to-face assessments and evaluate hospice eligibility as assigned.
- Serve as a clinical resource within community medical practices for early patient identification.
- Coordinate care transitions and collaborate with healthcare teams and community resources.
- Participate in interdisciplinary team meetings and case conferences regularly.
- Provide clinical support and education to hospice multidisciplinary teams.
- Supervise and consult on the care of palliative care patients in home-based programs.
- Engage in outreach to physicians to increase hospice referrals effectively.
- Assist in the design and development of palliative care programs and services.
Job Requirements
- Master's degree and completion of a New York State approved Nurse Practitioner program.
- Current licensure and certification as a Nurse Practitioner in a related specialty area.
- Completion of pharmacotherapeutic requirements for independent prescriptive privileges.
- Possession of National Provider Identifier (NPI) and DEA Registration Certificate.
- Minimum of five years of experience, including pain and symptom management expertise.
- Ability to build rapport with patients and thrive in fast-paced environments.
- Excellent written and verbal communication skills with strong leadership qualities.
- Proof of completed COVID-19 vaccination as per organizational requirements.
- Flexibility to participate in weekend, on-call coverage, and after-hours duties.
