Overview
- Lead the Care Coordination department, ensuring high-quality patient-centered services and effective team management within a healthcare organization.
- Provide supervision to Social Workers, Case Managers, and Patient Navigators in both pediatric and adult hospitals.
- Collaborate with the Director of Care Coordination to optimize departmental operations and achieve organizational goals.
- Ensure comprehensive care coordination assessments for patients, addressing discharge planning and progression barriers.
- Contribute to the development and continuous improvement of care coordination protocols and practices.
- Participate in hospital committees and task forces to support excellence in patient care.
- Promote diversity and inclusivity within the department, fostering a respectful and equitable work environment.
- Maintain awareness of current clinical issues and healthcare delivery changes to inform departmental decision-making.
Key Responsibilities & Duties
- Supervise and oversee the performance of Social Workers, Case Managers, and Patient Navigators to ensure quality service delivery.
- Conduct care coordination assessments for patients, documenting discharge plans and addressing barriers effectively.
- Support multidisciplinary rounds, providing education and resolving escalated issues as needed.
- Participate in complex case meetings, offering solutions for outlier patient barriers.
- Manage departmental operations, including scheduling, recruitment, and budget maintenance.
- Ensure timely delivery of psychosocial services to patients and their families during and after business hours.
- Engage in professional development activities, including continuing education and regional/national meetings.
- Establish relationships with healthcare professionals and community groups to coordinate services effectively.
Job Requirements
- Bachelor of Science in Nursing (BSN) or Master of Social Work from an accredited institution.
- Active RN license or LCSW certification, with preference for LCSW.
- Minimum of 5 years experience in medical social work or equivalent leadership role.
- Proven ability to manage departmental operations and staff performance effectively.
- Strong problem-solving skills and adherence to social work principles.
- Commitment to diversity, equity, and inclusion within the workplace.
- Knowledge of healthcare systems and patient care protocols.
