Overview
- Provide compassionate care to hospice patients, ensuring their comfort and dignity during end-of-life stages.
- Conduct thorough physical and psychosocial assessments to develop personalized care plans.
- Collaborate with interdisciplinary teams to address patient needs and optimize care delivery.
- Supervise and train home health aides, homemakers, and personal care workers.
- Request and implement physician orders to adapt care plans as required.
- Provide education and support to patients and families about hospice care processes.
- Ensure accurate documentation and data input to maintain comprehensive patient records.
- Participate in continuous improvement initiatives to enhance care quality and patient satisfaction.
Key Responsibilities & Duties
- Perform detailed assessments of patients' physical and emotional conditions.
- Develop and implement tailored nursing care plans based on individual needs.
- Supervise and evaluate the performance of home health aides and other care staff.
- Coordinate with physicians to update and modify care plans as necessary.
- Provide hands-on care and support to patients in their homes.

Job Requirements
- Associate of Science (AS) in Nursing or equivalent education.
- Registered Nurse (RN) license with a minimum of 2 years of clinical experience.
- Preferred experience in hospice, home care, or acute care settings.
- Strong communication, teaching, and interpersonal skills.
- Ability to work effectively within an interdisciplinary team environment.
