Job Description

Overview

  • Provide compassionate care to hospice patients, ensuring their comfort and dignity during end-of-life stages.
  • Conduct thorough physical and psychosocial assessments to develop personalized care plans.
  • Collaborate with interdisciplinary teams to address patient needs and optimize care delivery.
  • Supervise and train home health aides, homemakers, and personal care workers.
  • Request and implement physician orders to adapt care plans as required.
  • Provide education and support to patients and families about hospice care processes.
  • Ensure accurate documentation and data input to maintain comprehensive patient records.
  • Participate in continuous improvement initiatives to enhance care quality and patient satisfaction.

Key Responsibilities & Duties

  • Perform detailed assessments of patients' physical and emotional conditions.
  • Develop and implement tailored nursing care plans based on individual needs.
  • Supervise and evaluate the performance of home health aides and other care staff.
  • Coordinate with physicians to update and modify care plans as necessary.
  • Provide hands-on care and support to patients in their homes.
  • Educate patients and families on hospice care and available resources.
  • Ensure compliance with all regulatory and organizational standards.
  • Participate in team meetings to discuss patient progress and care strategies.

Job Requirements

  • Associate of Science (AS) in Nursing or equivalent education.
  • Registered Nurse (RN) license with a minimum of 2 years of clinical experience.
  • Preferred experience in hospice, home care, or acute care settings.
  • Strong communication, teaching, and interpersonal skills.
  • Ability to work effectively within an interdisciplinary team environment.
  • Proficiency in clinical data input and documentation.
