Job Description

Overview

  • Lead and supervise interdisciplinary hospice teams across multiple healthcare locations.
  • Ensure compliance with federal and state hospice care regulations.
  • Collaborate with hospital staff and organizational leadership for seamless patient care.
  • Provide clinical and business development leadership to enhance hospice services.
  • Oversee patient care planning and ensure quality and satisfaction metrics are met.
  • Participate in strategic planning and policy development for hospice care delivery.
  • Foster collaboration between hospital and hospice teams to support patient needs.
  • Manage hiring, scheduling, and performance evaluations of staff.

Key Responsibilities & Duties

  • Supervise interdisciplinary teams, providing guidance and support in clinical settings.
  • Conduct rounds, one-on-one supervision, and attend meetings across multiple sites.
  • Monitor staffing utilization and support business development efforts.
  • Ensure compliance with hospice care regulations and quality standards.
  • Lead quality improvement projects to enhance team performance.
  • Provide education on hospice principles and symptom management.
  • Collaborate with the Business Development Director to develop strategies.
  • Participate in hospice policy development and strategic planning.

Job Requirements

  • Bachelor of Science in Nursing (BSN) required; MSN or Master’s degree preferred.
  • Minimum of two years of hospice nursing experience required.
  • Supervisory or leadership experience preferred.
  • CHPN certification required or to be completed within one year of hire.
  • Strong clinical knowledge of hospice regulations and patient care.
  • Valid New York State RN licensure and CPR-BLS certification.
  • Ability to manage priorities and work flexible hours.
  • Valid NYS Driver's license preferred.
