Overview
- Lead and supervise interdisciplinary hospice teams across multiple healthcare locations.
- Ensure compliance with federal and state hospice care regulations.
- Collaborate with hospital staff and organizational leadership for seamless patient care.
- Provide clinical and business development leadership to enhance hospice services.
- Oversee patient care planning and ensure quality and satisfaction metrics are met.
- Participate in strategic planning and policy development for hospice care delivery.
- Foster collaboration between hospital and hospice teams to support patient needs.
- Manage hiring, scheduling, and performance evaluations of staff.
Key Responsibilities & Duties
- Supervise interdisciplinary teams, providing guidance and support in clinical settings.
- Conduct rounds, one-on-one supervision, and attend meetings across multiple sites.
- Monitor staffing utilization and support business development efforts.
- Ensure compliance with hospice care regulations and quality standards.
- Lead quality improvement projects to enhance team performance.
- Provide education on hospice principles and symptom management.
- Collaborate with the Business Development Director to develop strategies.
- Participate in hospice policy development and strategic planning.
Job Requirements
- Bachelor of Science in Nursing (BSN) required; MSN or Master’s degree preferred.
- Minimum of two years of hospice nursing experience required.
- Supervisory or leadership experience preferred.
- CHPN certification required or to be completed within one year of hire.
- Strong clinical knowledge of hospice regulations and patient care.
- Valid New York State RN licensure and CPR-BLS certification.
- Ability to manage priorities and work flexible hours.
- Valid NYS Driver's license preferred.
