Job Description

Overview

  • Provide social work services in an outpatient psychiatric setting, focusing on biopsychosocial assessments and care coordination.
  • Collaborate with multidisciplinary teams to develop and implement patient treatment plans.
  • Assist patients with navigating community resources and protective services processes.
  • Offer counseling services, including individual, marriage, and parental therapy.
  • Coordinate post-discharge services, including home care and equipment needs.
  • Document patient interactions and care plans to ensure continuity of care.
  • Engage in collaborative care with other healthcare professionals and community resources.
  • Participate in care conferences and contribute to treatment planning discussions.

Key Responsibilities & Duties

  • Conduct comprehensive biopsychosocial assessments for patients in psychiatric care.
  • Coordinate services with community programs, including Medicaid, TANF, and food stamps.
  • Assist patients with protective services such as guardianship and emergency detention.
  • Provide therapy and counseling tailored to individual and family needs.
  • Refer patients to appropriate facilities and resources for rehabilitation and long-term care.
  • Document patient progress and interactions in the charting system.
  • Collaborate with healthcare teams to ensure effective care delivery.
  • Participate in the development and implementation of treatment plans.

Job Requirements

  • Master of Social Work degree required.
  • Minimum of 2 years of experience in social work, preferably in psychiatric settings.
  • Certifications required: BLS and LCSW.
  • Proficiency in charting systems and documentation practices.
  • Knowledge of community programs such as Medicaid, TANF, and food stamps.
  • Familiarity with protective services processes, including guardianship and emergency detention.
  • Strong interpersonal and communication skills for patient counseling and team collaboration.
