Overview
- Provide social work services in an outpatient psychiatric setting, focusing on biopsychosocial assessments and care coordination.
- Collaborate with multidisciplinary teams to develop and implement patient treatment plans.
- Assist patients with navigating community resources and protective services processes.
- Offer counseling services, including individual, marriage, and parental therapy.
- Coordinate post-discharge services, including home care and equipment needs.
- Document patient interactions and care plans to ensure continuity of care.
- Engage in collaborative care with other healthcare professionals and community resources.
- Participate in care conferences and contribute to treatment planning discussions.
Key Responsibilities & Duties
- Conduct comprehensive biopsychosocial assessments for patients in psychiatric care.
- Coordinate services with community programs, including Medicaid, TANF, and food stamps.
- Assist patients with protective services such as guardianship and emergency detention.
- Provide therapy and counseling tailored to individual and family needs.
- Refer patients to appropriate facilities and resources for rehabilitation and long-term care.
- Document patient progress and interactions in the charting system.
- Collaborate with healthcare teams to ensure effective care delivery.
- Participate in the development and implementation of treatment plans.
Job Requirements
- Master of Social Work degree required.
- Minimum of 2 years of experience in social work, preferably in psychiatric settings.
- Certifications required: BLS and LCSW.
- Proficiency in charting systems and documentation practices.
- Knowledge of community programs such as Medicaid, TANF, and food stamps.
- Familiarity with protective services processes, including guardianship and emergency detention.
- Strong interpersonal and communication skills for patient counseling and team collaboration.
