Job Description

Overview

  • Provide nursing care and support to clients in the ACT program, focusing on behavioral health populations.
  • Conduct field-based work, visiting clients in their homes and shelters, ensuring accessibility and comprehensive care.
  • Perform initial nursing assessments and ongoing health monitoring to address physical health needs.
  • Administer medications as prescribed, ensuring adherence to policies and monitoring for side effects.
  • Collaborate with psychiatrists, PNPs, and external healthcare providers to coordinate client care.
  • Provide health education and training to clients and staff to promote continuous health support.
  • Participate in interdisciplinary team meetings, contributing medical expertise and facilitating discussions.
  • Supervise and train part-time nursing staff, ensuring compliance with ACT program standards.
  • Respond to medical emergencies and provide after-hours coverage on a rotating basis.

Key Responsibilities & Duties

  • Carry a caseload, conducting initial assessments and developing treatment plans based on client needs.
  • Perform weekly visits to clients in shelters or homes, administering prescribed care and follow-ups.
  • Coordinate and deliver medications, ensuring proper documentation and adherence to internal policies.
  • Make referrals, schedule appointments, and accompany clients as needed for medical care.
  • Provide advocacy and follow-up care for hospitalized clients to promote recovery.
  • Audit program charts monthly to ensure compliance with OMH standards of care.
  • Train staff in health practices and facilitate health education groups for clients.
  • Monitor lab results, communicating changes to medical professionals for appropriate action.
  • Supervise part-time nurses, overseeing their assignments and ensuring skill development.

Job Requirements

  • Bachelor of Science in Nursing (BSN) with active New York State RN registration.
  • Minimum of 1 year of nursing experience, preferably in behavioral health settings.
  • Strong knowledge of nursing assessments, medication administration, and health monitoring protocols.
  • Ability to work independently in field-based settings, providing care in clients' homes or shelters.
  • Effective communication skills to liaise with healthcare providers and community organizations.
  • Proficiency in documentation and compliance with regulatory standards.
  • Capability to supervise and train nursing staff, ensuring adherence to program requirements.
  • Flexibility to provide after-hours emergency coverage as part of a rotating schedule.
