Overview

  • Provide high-quality patient care in a multicultural primary care clinic setting.
  • Collaborate with a dedicated healthcare team to improve patient outcomes and satisfaction.
  • Participate in quality improvement initiatives to enhance clinical performance and compliance.
  • Contribute to policy development and standards of patient care through committee participation.
  • Support team-based approaches within a Patient-Centered Medical Home framework.

Key Responsibilities & Duties

  • Deliver professional healthcare services within the clinic environment.
  • Work closely with clinical and non-clinical staff to enhance patient care procedures.
  • Participate in phone on-call rotations as part of the clinic's operational schedule.
  • Collaborate with specialists in nearby neighborhoods to provide integrated care.
  • Engage in quality improvement efforts to maintain clinical performance standards.
  • Contribute to the development of patient care policies and standards.
  • Support the clinic's goals of achieving population health and financial sustainability.
  • Ensure compliance with privacy and security policies regarding patient information.

Job Requirements

  • Master of Science in Nursing from an accredited program.
  • Valid New York Medical License and active DEA certification.
  • Board certification as Nurse Practitioner (ANP, AGNP, FNP, ENP).
  • Minimum of 2 years of clinical experience in primary care settings.
  • Fluency in Spanish for conversational and medical communication.
  • Experience with Article 28 and Patient-Centered Medical Home preferred.
  • Strong interpersonal, communication, organizational, and collaborative skills.
  • Commitment to improving clinical outcomes and patient satisfaction.
