Job Description

Overview

  • Provide comprehensive primary care services to patients, focusing on holistic health and wellness.
  • Collaborate with multidisciplinary teams to deliver patient-centered care and achieve health outcomes.

Key Responsibilities & Duties

  • Diagnose, treat, and manage acute and chronic illnesses in older adult patients.
  • Lead a care team to coordinate and deliver comprehensive patient care.
  • Collaborate with specialists and hospitalists to ensure continuity of care.
  • Engage in patient education and health promotion activities.
  • Document patient interactions and care plans using efficient note-taking practices.
  • Participate in quality improvement initiatives and adhere to regulatory standards.
  • Recruit patients and providers to support center growth and mission.
  • Supervise and collaborate with Advanced Practice Practitioners (APPs) as needed.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics required.
  • Active state medical license or eligibility to obtain one within the required timeframe.
  • Current DEA certification for controlled substances is mandatory.
  • Basic Life Support (BLS) certification required within 90 days of employment.
  • Proficiency in Microsoft Office Suite and healthcare-related software.
  • Strong commitment to patient-centered care and mission-driven healthcare delivery.
  • Ability to travel locally and nationally for training and recruitment purposes.
