Key Responsibilities & Duties

- Develop personalized care plans, setting short and long-term health goals with patients.
- Lead care teams, including medical assistants and care coordinators, to optimize patient outcomes.
- Ensure compliance with clinical guidelines and participate in quality improvement initiatives.
- Engage in community outreach and patient recruitment activities to expand service impact.

- Document patient interactions and care plans efficiently using electronic health records.

Job Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board certification in Internal Medicine, Family Medicine, or Geriatrics required.
- Active state medical license or eligibility to obtain one within the required timeframe.
- Minimum of 1 year of experience; 5 years preferred in primary care settings.
- Proficiency in electronic health record systems and Microsoft Office Suite.
- Strong commitment to patient-centered care and service orientation.
- Basic Life Support (BLS) certification required within 90 days of employment.
- Ability to work onsite Monday through Friday, adhering to structured schedules.
