Licensed Practical Nurse

Overview

• Provide direct nursing care in compliance with standards and regulations.
• Develop and implement individualized nursing care plans for clients.
• Coordinate acute and ongoing health services based on client needs.
• Monitor health status, labs, and treatment efficacy, ensuring timely communication.
• Maintain accurate medical records and documentation reflecting outcomes.
• Provide health education to clients and their support systems.
• Train staff on nursing tasks, medication administration, and infection control.
• Participate actively in interdisciplinary team discussions and planning.
• Ensure compliance with safety protocols and emergency response procedures.

Key Responsibilities & Duties

• Conduct physical examinations and nursing assessments for clients.
• Update nursing care plans annually or upon significant condition changes.
• Supervise and schedule LPN staff hours effectively.
• Provide ongoing training and support to staff in nursing practices.
• Maintain electronic health records and coordinate care with external providers.
• Ensure proper documentation of training attendance and testing results.
• Respond swiftly to emergencies, performing necessary procedures.
• Participate in recruitment and evaluation of nursing staff candidates.
• Collaborate with the Director of Nursing on operational improvements.

Job Requirements

• Active, unrestricted New York State LPN license required.
• Bachelor of Science in Nursing preferred.
• Minimum of 1 year of nursing experience; managerial experience preferred.
• Proficiency in Microsoft Office Suite and electronic health record systems.
• Strong verbal and written communication skills essential.
• Ability to work independently and within a team environment.
• Physical stamina to perform nursing duties effectively throughout shifts.
