Key Responsibilities & Duties

Job Requirements
- Bachelor of Science in Nursing (BSN) and current Registered Nurse (RN) license in New York State.
- Certified Diabetes Care and Education Specialist (CDCES) certification or eligibility preferred.
- Minimum of 4 years of nursing experience, with a focus on diabetes education or chronic disease management.
- Strong communication and teaching skills to effectively educate and counsel patients.
- Experience in community health or outpatient settings is highly desirable.
- Proficiency in electronic health records (EHR) systems for accurate documentation.
- Bilingual proficiency in Creole and English is highly preferred.
