Overview

Key Responsibilities & Duties

  • Provide comprehensive education and counseling to patients on diabetes management, including nutrition, medication, and lifestyle adjustments.
  • Develop and implement individualized care plans in collaboration with healthcare providers and care teams.
  • Monitor patient progress, making necessary adjustments to care strategies to achieve health goals.
  • Conduct educational sessions, both group and individual, focusing on prevention and disease management.
  • Act as a resource for staff, providing guidance on best practices in diabetes care.
  • Ensure accurate and thorough documentation in electronic health records for all patient interactions.
  • Engage in community outreach initiatives to promote diabetes awareness and prevention.
  • Participate in quality improvement activities to enhance patient care and outcomes.

Job Requirements

  • Bachelor of Science in Nursing (BSN) and current Registered Nurse (RN) license in New York State.
  • Certified Diabetes Care and Education Specialist (CDCES) certification or eligibility preferred.
  • Minimum of 4 years of nursing experience, with a focus on diabetes education or chronic disease management.
  • Strong communication and teaching skills to effectively educate and counsel patients.
  • Experience in community health or outpatient settings is highly desirable.
  • Proficiency in electronic health records (EHR) systems for accurate documentation.
  • Bilingual proficiency in Creole and English is highly preferred.
