Overview
- Provide comprehensive primary care services to a diverse patient population, ensuring compassionate, unbiased, and evidence-based care delivery.
- Collaborate within a multidisciplinary team to promote patient-centered care and wellness.
- Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan.
- Document care promptly and accurately in the electronic health record system (EPIC).
- Participate in quality improvement initiatives and professional development activities.
- Work flexible schedules including morning, evening, and optional Saturday shifts.
- Engage in community-based healthcare with a focus on accessible, patient-centered services.
- Specialize in HIV care and transgender healthcare, enhancing patient outcomes.
Key Responsibilities & Duties
- Conduct thorough patient evaluations, screenings, and diagnoses to develop effective treatment plans.
- Provide primary care services in scheduled appointments, ensuring timely follow-up on patient needs.
- Collaborate with healthcare professionals to coordinate patient care within a team-based model.
- Make appropriate referrals to internal and external specialists as needed.
- Participate in case conferences and quality improvement initiatives to enhance care delivery.
- Engage in continuing medical education to maintain professional competency.
- Provide care that aligns with the organization’s mission of accessible healthcare for all.
- Ensure accurate and timely documentation of patient interactions in the electronic health record.
Job Requirements
- Doctoral degree (DO) with New York State Certification/Licensure (MD, DO, NP).
- Current NYS Registration and DEA Controlled Substances Registration Certificate.
- Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease.
- Minimum of 2 years of primary care experience; 3 years preferred.
- Experience in HIV care and transgender healthcare is highly desirable.
- Proficiency in electronic health record systems such as EPIC.
- Ability to work effectively with individuals from diverse backgrounds.
- Bilingual proficiency (Spanish/English) is a plus.
