Overview
- Provide comprehensive primary care services to a diverse patient population in a community-focused healthcare setting.
- Deliver patient-centered care with a focus on respect, compassion, and evidence-based practices.
- Collaborate with multidisciplinary teams to ensure coordinated and effective patient care.
- Participate in quality improvement initiatives and professional development programs.
- Work flexible schedules, including morning, evening, and optional weekend shifts.
- Engage with patients in scheduled appointments and provide timely follow-up care.
- Document patient interactions and care plans accurately in the electronic health record system.

Key Responsibilities & Duties
- Conduct patient assessments, screenings, and examinations to diagnose and treat medical conditions.
- Provide primary care services across the lifespan, including preventive care and chronic disease management.
- Collaborate with healthcare professionals to develop and implement patient care plans.
- Refer patients to appropriate internal or external specialists as needed.
- Participate in case conferences and quality improvement initiatives to enhance care delivery.
- Maintain accurate and timely documentation in the electronic health record system.
- Engage in continuing medical education and professional development opportunities.
- Support the organization's mission and advocate for community health initiatives.
Job Requirements
- Doctoral degree in Medicine (MD/DO) or Nurse Practitioner certification/licensure.
- Current New York State registration and DEA Controlled Substances Registration Certificate.
- Board certification or eligibility in Family Medicine, Internal Medicine, or Infectious Disease.
- Minimum of 2 years of primary care experience; 3 years preferred.
- Experience in HIV and transgender care is highly desirable.
- Proficiency in working with diverse patient populations and commitment to accessible healthcare.
- Strong collaboration skills within a team-based care model.
- Bilingual proficiency in Spanish/English is a plus.
