Job Description

Overview

  • Provide comprehensive primary care services to a diverse patient population in a community-focused healthcare setting.
  • Deliver patient-centered care with a focus on respect, compassion, and evidence-based practices.
  • Collaborate with multidisciplinary teams to ensure coordinated and effective patient care.
  • Participate in quality improvement initiatives and professional development programs.
  • Work flexible schedules, including morning, evening, and optional weekend shifts.
  • Engage with patients in scheduled appointments and provide timely follow-up care.
  • Document patient interactions and care plans accurately in the electronic health record system.
  • Contribute to the mission of providing accessible healthcare to underserved communities.

Key Responsibilities & Duties

  • Conduct patient assessments, screenings, and examinations to diagnose and treat medical conditions.
  • Provide primary care services across the lifespan, including preventive care and chronic disease management.
  • Collaborate with healthcare professionals to develop and implement patient care plans.
  • Refer patients to appropriate internal or external specialists as needed.
  • Participate in case conferences and quality improvement initiatives to enhance care delivery.
  • Maintain accurate and timely documentation in the electronic health record system.
  • Engage in continuing medical education and professional development opportunities.
  • Support the organization's mission and advocate for community health initiatives.

Job Requirements

  • Doctoral degree in Medicine (MD/DO) or Nurse Practitioner certification/licensure.
  • Current New York State registration and DEA Controlled Substances Registration Certificate.
  • Board certification or eligibility in Family Medicine, Internal Medicine, or Infectious Disease.
  • Minimum of 2 years of primary care experience; 3 years preferred.
  • Experience in HIV and transgender care is highly desirable.
  • Proficiency in working with diverse patient populations and commitment to accessible healthcare.
  • Strong collaboration skills within a team-based care model.
  • Bilingual proficiency in Spanish/English is a plus.
