Overview

  • Deliver high-quality, patient-centered primary care services to a diverse population.

Key Responsibilities & Duties

  • Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan.
  • Provide timely follow-up on patient needs, test results, and referrals.
  • Document all care promptly in the electronic health record system.
  • Collaborate with healthcare professionals to ensure coordinated patient care.
  • Participate in case conferences and quality improvement initiatives.
  • Engage in continuing medical education and professional development activities.
  • Provide care that aligns with the organization’s mission and values.
  • Make appropriate internal and external referrals for specialized care.

Job Requirements

  • Doctoral degree (DO, MD, or NP) with New York State Certification/Licensure.
  • Current NYS Registration and DEA Controlled Substances Registration Certificate.
  • Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease.
  • Minimum of 2 years of primary care experience; 3 years preferred.
  • Experience in HIV and transgender care is highly desirable.
  • Strong collaboration skills in a team-based model.
  • Commitment to accessible, patient-centered primary care.
  • Bilingual proficiency (Spanish/English) is a plus.
