Overview
- Provide comprehensive women’s health services, including preventive care, contraception, and chronic disease management, in a field-based setting.
- Collaborate with interdisciplinary teams to deliver patient-centered care tailored to diverse populations.
- Work closely with patients and caregivers to develop and implement personalized care plans.
- Conduct well-woman exams and provide education on preventive health needs.
- Insert and manage long-acting reversible contraception devices, ensuring patient comfort and understanding.
- Engage in field-based care, meeting patients in various locations to enhance accessibility.
- Utilize evidence-based guidelines to manage complex medical and mental health conditions.
- Promote disease education and evidence-based healthcare services to improve patient outcomes.
Key Responsibilities & Duties
- Perform comprehensive assessments of patients’ healthcare needs and develop tailored care plans.
- Coordinate care transitions and facilitate communication between patients, caregivers, and healthcare teams.
- Conduct caregiver assessments to ensure holistic care management.
- Educate patients on available contraception options and provide prescriptions as needed.
- Insert intrauterine devices and other implanted contraception devices with precision and care.
- Monitor and manage chronic medical and mental health conditions using evidence-based practices.
- Collaborate with interdisciplinary teams to ensure high-quality outcomes for patients and families.
- Maintain accurate and detailed patient records to support effective care delivery.
Job Requirements
- Master of Science in Nursing with certification as WHNP or FNP.
- Minimum of 1 year of experience in women’s health; 2 years preferred.
- Proficiency in conducting well-woman exams and managing contraception.
- Experience in field-based or home care settings is advantageous.
- Strong interpersonal skills to engage effectively with patients and caregivers.
- Knowledge of evidence-based guidelines for chronic disease management.
- Ability to work independently and adapt to flexible schedules.
