Overview
- Provide comprehensive nursing care to hospice patients, ensuring comfort and dignity during end-of-life stages.
- Conduct physical and psychosocial assessments to develop individualized care plans addressing patient needs.
- Collaborate with interdisciplinary teams to deliver holistic and compassionate care.
- Supervise and train home health aides and personal care workers in patient homes.
- Request and manage physician orders for adjustments in patient care plans.
- Ensure accurate documentation and proficient input of clinical data.
- Support patients and families through effective communication and education.
- Maintain compliance with hospice care standards and regulations.
Key Responsibilities & Duties
- Perform initial and ongoing assessments of hospice patients to determine care needs.
- Develop and implement personalized nursing care plans based on patient conditions.
- Coordinate with physicians and interdisciplinary teams to ensure optimal care delivery.
- Train and evaluate home health aides and personal care workers in patient care techniques.

Job Requirements
- Associate of Applied Science (AAS) in Nursing or related field required.
- Minimum 2 years of clinical experience, preferably in hospice, home care, or acute care settings.
- Strong ability to work within interdisciplinary team environments.
- Proficient in clinical data input and documentation.
- Excellent teaching, communication, and listening skills.
- Knowledge of hospice care standards and practices preferred.
