Job Description

Overview

  • Provide comprehensive nursing care to hospice patients, ensuring comfort and dignity during end-of-life stages.
  • Conduct physical and psychosocial assessments to develop individualized care plans addressing patient needs.
  • Collaborate with interdisciplinary teams to deliver holistic and compassionate care.
  • Supervise and train home health aides and personal care workers in patient homes.
  • Request and manage physician orders for adjustments in patient care plans.
  • Ensure accurate documentation and proficient input of clinical data.
  • Support patients and families through effective communication and education.
  • Maintain compliance with hospice care standards and regulations.

Key Responsibilities & Duties

  • Perform initial and ongoing assessments of hospice patients to determine care needs.
  • Develop and implement personalized nursing care plans based on patient conditions.
  • Coordinate with physicians and interdisciplinary teams to ensure optimal care delivery.
  • Train and evaluate home health aides and personal care workers in patient care techniques.
  • Monitor patient progress and adjust care plans as necessary.
  • Provide education and support to patients and their families regarding hospice care.
  • Maintain accurate and timely documentation of patient care activities.
  • Ensure adherence to all hospice care policies and procedures.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or related field required.
  • Minimum 2 years of clinical experience, preferably in hospice, home care, or acute care settings.
  • Strong ability to work within interdisciplinary team environments.
  • Proficient in clinical data input and documentation.
  • Excellent teaching, communication, and listening skills.
  • Knowledge of hospice care standards and practices preferred.
  • Ability to supervise and train home health aides effectively.
