Key Responsibilities & Duties

  • Conduct thorough patient assessments using the Uniform Assessment System (UAS) framework.
  • Develop individualized care plans in collaboration with healthcare teams.
  • Ensure accurate documentation and compliance with healthcare regulations.

Job Requirements

  • Associate of Science (AS) in Nursing or equivalent qualification required.
  • Minimum of 2 years of nursing experience, preferably in assessment roles.
  • Proficiency in using the Uniform Assessment System (UAS) framework.
  • Strong interpersonal and communication skills for patient and team interactions.
  • Ability to work effectively in a hybrid work environment.
