Overview
- Serve as an Admissions Nurse for a hospice program, ensuring timely and safe patient admissions.
- Conduct fieldwork for admissions and assessments in designated boroughs using public transportation or rideshare services.
- Collaborate with interdisciplinary hospice teams to ensure comprehensive patient care.
- Manage high-tech and complex referrals within the Access Center, monitoring referral lists.
- Work under the guidance of the Access Nurse Team Leader to streamline admissions processes.
- Perform evaluations and verify orders prior to patient admission to the hospice program.
- Ensure adherence to hospice program standards and protocols during patient intake.
- Maintain accurate documentation and communication with all involved disciplines.
Key Responsibilities & Duties
- Review and evaluate patient admissions to the hospice program, ensuring compliance with criteria.
- Perform field admissions and assessments, managing a caseload effectively.
- Coordinate with Access Coordinators to facilitate timely patient admissions.
- Communicate with hospice team members, including nurses, physicians, and social workers.
- Monitor referral lists and manage complex referrals within the Access Center.
- Verify written and verbal orders for evaluation prior to patient admission.
- Provide excellent customer service and maintain professional communication standards.
- Participate in weekend shifts as required, ensuring consistent patient care.
Job Requirements
- Graduate from an accredited school of Nursing with an Associate of Applied Science (AAS) degree.
- Hold a valid NY State RN license; BSN preferred but not required.
- Minimum of 2 years of experience; 6 years preferred in acute care, ER, critical care, oncology, or long-term care.
- Proficiency in PC skills and strong written and oral communication abilities.
- Demonstrated customer service skills and ability to work collaboratively in a team environment.
- Ability to travel within designated boroughs using public transportation or rideshare services.
- Flexibility to work 11 AM to 7 PM shifts and one weekend per month.
- Bilingual Spanish proficiency required for Manhattan and Bronx assignments.
