Overview

  • Lead clinical operations for a home healthcare agency, ensuring high-quality patient care and compliance with regulations.
  • Coordinate patient care needs, collaborating with healthcare professionals to develop individualized care plans.
  • Provide oversight to clinical field staff, ensuring effective case management and documentation accuracy.
  • Serve as a resource for agency inquiries, promoting excellent customer relations and service quality.
  • Participate in quality improvement initiatives and staff orientation processes.
  • Maintain confidentiality and demonstrate cultural sensitivity in all interactions.
  • Utilize community resources to address patient care needs effectively.
  • Engage in on-call supervisor rotation to support agency operations.
  • Ensure compliance with state and federal regulations, enhancing organizational standards.

Key Responsibilities & Duties

  • Screen and assess home healthcare needs for referred patients, coordinating intake processes.
  • Collaborate with physicians and discharge planners to establish appropriate care levels.
  • Develop comprehensive care plans aligned with medical orders and insurance guidelines.
  • Order necessary medical supplies and services, ensuring proper documentation.
  • Assign and schedule visits for field staff, ensuring timely service delivery.
  • Conduct case conferences to ensure effective case management and discharge planning.
  • Review admission and recertification documentation for care plan appropriateness.
  • Participate in performance improvement activities to enhance service quality.
  • Provide clear direction to support staff for continuity of care.

Job Requirements

  • License to practice as a registered nurse; BSN preferred.
  • Minimum three years of nursing experience; two years in home health preferred.
  • Knowledge of OASIS and clinical documentation standards.
  • Effective verbal and written communication skills.
  • Strong problem-solving and organizational abilities.
  • Commitment to customer relations and quality service.
  • Ability to manage multiple responsibilities and set priorities effectively.
  • Demonstrated knowledge of reimbursement and billing practices.
