Overview

  • Deliver high-quality nursing care to patients in a home health setting, ensuring their well-being and recovery.
  • Educate patients and families on health habits and prevention strategies to promote long-term wellness.
  • Collaborate with healthcare professionals to develop and implement effective care plans.
  • Utilize advanced nursing skills to provide compassionate, patient-centered care with minimal supervision.
  • Follow health and safety guidelines consistently to ensure patient and staff safety.
  • Maintain accurate and timely documentation using provided tools and technology.
  • Support the organization’s mission to deliver culturally competent, high-quality care.
  • Participate in training and orientation programs to enhance professional skills.

Key Responsibilities & Duties

  • Provide visiting home health services, including patient admissions, re-certifications, and discharges.
  • Develop and coordinate physician care plans with healthcare professionals involved in patient cases.
  • Complete comprehensive OASIS D1 assessments for admissions, transfers, and discharges.
  • Ensure compliance with Department of Health and government regulations in all services provided.
  • Document patient care activities within the required timeframe using a tablet.
  • Provide culturally sensitive care that aligns with agency standards and patient needs.
  • Report daily activities and patient progress to the Clinical Manager.
  • Maintain professional relationships with patients and their families to ensure effective communication.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent education.
  • Valid Nursing license with proven experience as a Registered Nurse.
  • Minimum of 1 year of experience in acute care, hospital, or home care settings.
  • Proficiency in completing OASIS assessments and using Homecare Homebase (HCHB) software.
  • Ability to drive and access to a reliable vehicle for patient visits.
  • Strong communication skills and ability to work independently.
  • Compassionate and patient-focused approach to nursing care.
