Key Responsibilities & Duties
- Provide visiting home health services, including patient admissions, re-certifications, and discharges.
- Develop and coordinate physician care plans with healthcare professionals involved in patient cases.
- Complete comprehensive OASIS D1 assessments for admissions, transfers, and discharges.
- Ensure compliance with Department of Health and government regulations in all services provided.
- Document patient care activities within the required timeframe using a tablet.
- Provide culturally sensitive care that aligns with agency standards and patient needs.
- Report daily activities and patient progress to the Clinical Manager.
- Maintain professional relationships with patients and their families to ensure effective communication.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent education.
- Valid Nursing license with proven experience as a Registered Nurse.
- Minimum of 1 year of experience in acute care, hospital, or home care settings.
- Proficiency in completing OASIS assessments and using Homecare Homebase (HCHB) software.
- Ability to drive and access to a reliable vehicle for patient visits.
- Strong communication skills and ability to work independently.
- Compassionate and patient-focused approach to nursing care.
