Overview
- Lead and mentor a team of hospice nurses, ensuring high-quality care and adherence to program goals.
- Provide professional development opportunities and guidance to nursing staff to foster growth and excellence.
- Balance fieldwork and administrative responsibilities, including case management and staff supervision.
- Collaborate with the Hospice Nurse Manager to implement strategies and achieve organizational objectives.
- Ensure compliance with hospice and palliative care policies and standards.
- Maintain effective communication with team members and stakeholders to enhance service delivery.
- Contribute to the development of a supportive and collaborative work environment.
- Participate in fieldwork within the Bronx territory, providing direct patient care and supervision.
Key Responsibilities & Duties
- Carry a reduced patient caseload, ensuring comprehensive and compassionate hospice care.
- Mentor and supervise nursing staff, fostering professional growth and development.
- Assist with staff orientation and training to ensure readiness and competence.
- Provide leadership and guidance to ensure adherence to hospice program policies and standards.
- Collaborate with the Hospice Nurse Manager on operational and strategic initiatives.
- Perform fieldwork and case management duties within the Bronx territory.
- Participate in rotating weekend schedules to ensure continuous patient care.
- Utilize Microsoft Word, Outlook, and Windows for documentation and communication tasks.
Job Requirements
- Associate of Applied Science (AAS) in Nursing; BSN preferred.
- Minimum of two years of nursing experience; four years preferred.
- New York State RN licensure required; CHPN certification preferred.
- Proficiency in Microsoft Word, Outlook, and Windows applications.
- Strong leadership and mentoring skills to supervise hospice nursing staff effectively.
- Ability to perform fieldwork and case management within the Bronx territory.
- Excellent communication and organizational skills to manage team dynamics and patient care.
