Key Responsibilities & Duties

  • Conduct home health visits, including patient admissions, re-certifications, and discharges, ensuring high-quality care.
  • Develop and coordinate Physician Plans of Care in collaboration with healthcare professionals.
  • Document patient care activities accurately and promptly using provided technology tools.
  • Perform OASIS D1 assessments for admissions, re-certifications, transfers, and discharges.
  • Ensure compliance with Department of Health regulations and organizational standards.
  • Deliver culturally competent care that aligns with the organization’s mission and values.
  • Collaborate with the Clinical Manager and interdisciplinary team to achieve patient recovery goals.
  • Provide patient and family education on health management and preventive care strategies.

Job Requirements

  • Valid Nursing license and proven experience as a registered nurse.
  • Associate of Applied Science (AAS) degree in Nursing or equivalent qualification.
  • Experience in acute care, hospital settings, or home care is essential.
  • Proficiency in completing comprehensive OASIS assessments is required.
  • Pediatric experience is preferred but not mandatory for this role.
  • Must possess a reliable vehicle and valid driver’s license for travel.
  • Experience with Homecare Homebase (HCHB) software is an advantage.
  • Strong communication and organizational skills to manage patient care effectively.
