Key Responsibilities & Duties
- Conduct home health visits, including patient admissions, re-certifications, and discharges.
- Develop and implement physician-directed care plans tailored to patient needs.
- Complete OASIS D1 assessments for admissions and transitions within required timelines.
- Ensure compliance with healthcare regulations and documentation standards.
- Educate patients on preventive care and healthy lifestyle practices.
- Report daily to the Clinical Manager and manage a dedicated caseload.
- Utilize Homecare Homebase software for efficient patient management.
- Support the organization’s mission to deliver high-quality, culturally sensitive care.
Job Requirements
- Valid Nursing license and proven experience as a Registered Nurse.
- Associate of Applied Science (AAS) degree in Nursing required.
- Experience in acute care, hospital settings, or home care preferred.
- Proficiency in conducting OASIS assessments and maintaining documentation standards.
- Pediatric care experience and ability to drive with a personal vehicle.
- Familiarity with Homecare Homebase software is advantageous.
- Strong communication skills and ability to work independently.
