Key Responsibilities & Duties

  • Conduct home health visits, including patient admissions, re-certifications, and discharges.
  • Develop and implement physician-directed care plans tailored to patient needs.
  • Complete OASIS D1 assessments for admissions and transitions within required timelines.
  • Ensure compliance with healthcare regulations and documentation standards.
  • Educate patients on preventive care and healthy lifestyle practices.
  • Report daily to the Clinical Manager and manage a dedicated caseload.
  • Utilize Homecare Homebase software for efficient patient management.
  • Support the organization’s mission to deliver high-quality, culturally sensitive care.

Job Requirements

  • Valid Nursing license and proven experience as a Registered Nurse.
  • Associate of Applied Science (AAS) degree in Nursing required.
  • Experience in acute care, hospital settings, or home care preferred.
  • Proficiency in conducting OASIS assessments and maintaining documentation standards.
  • Pediatric care experience and ability to drive with a personal vehicle.
  • Familiarity with Homecare Homebase software is advantageous.
  • Strong communication skills and ability to work independently.
