Job Description

Overview

  • Serve as a Patient Care Liaison, ensuring seamless transitions for patients into nursing home settings while collaborating with healthcare professionals.
  • Facilitate patient admissions by coordinating with physicians, social workers, and case managers to ensure optimal care delivery.
  • Engage in outreach activities to promote services to hospitals and community organizations, fostering strong professional relationships.
  • Provide compassionate support to patients and families during transitions, addressing concerns and ensuring satisfaction.
  • Contribute to marketing initiatives, including social media campaigns, to enhance visibility and community engagement.
  • Participate in organizing educational programs and community relations events to strengthen organizational presence.
  • Ensure adherence to healthcare policies and procedures, maintaining compliance and high standards of care.

Key Responsibilities & Duties

  • Conduct daily hospital visits to establish and maintain relationships with healthcare professionals, including social workers and case managers.
  • Act as a key liaison between healthcare providers and agency staff, ensuring effective communication and coordination.
  • Develop and implement marketing strategies to promote services and enhance community outreach.
  • Organize and deliver presentations to community groups and healthcare organizations, showcasing services and benefits.

Job Requirements

  • Associate of Arts (AA) degree or equivalent educational background required.
  • Minimum of 1 year of experience in patient care liaison roles or similar settings.
  • Demonstrated ability to network with healthcare providers and community organizations effectively.
  • Knowledge of communication principles, marketing strategies, and community relations practices.
  • Strong organizational skills and ability to present ideas to diverse audiences confidently.
  • Excellent oral and written communication skills for professional interactions.
