Nurse Practitioner

Overview

• Provide primary care services to patients in an assisted living facility.
• Diagnose and manage acute and chronic conditions with a patient-centered approach.
• Prescribe medications and handle prescription refills as needed.
• Communicate effectively with patients, families, and facility personnel.
• Order and interpret diagnostic tests, including imaging and lab work.
• Document patient histories and treatments in electronic medical records (EMR).
• Work on-site within the wellness suite of the assisted living facility.
• Collaborate with other healthcare professionals to ensure comprehensive care.

Key Responsibilities & Duties

• Deliver in-person primary care services to patients in a wellness suite setting.
• Diagnose, treat, and manage various health conditions effectively.
• Prescribe medications and manage prescription refills as required.
• Communicate with patients, families, and facility staff to ensure coordinated care.
• Order diagnostic tests and interpret results to guide treatment plans.
• Maintain accurate and detailed patient records using EMR systems.
• Participate in case discussions with facility personnel and specialists.
• Ensure compliance with healthcare regulations and standards.

Job Requirements

• Bachelor of Science in Nursing (BSN) degree required.
• Active unrestricted New York state Nurse Practitioner license.
• Minimum two years of clinical experience; three years preferred.
• DEA certification and PECOS enrollment required.
• Knowledge of eClinicalWorks EMR system preferred.
• Medicare/Medicaid approved provider status required.
• Coalition for Affordable Quality Healthcare (CAQH) setup necessary.
• Basic Cardiac Life Support (BCLS) certification required.
