Key Responsibilities & Duties

• Provide psychosocial support to hospice patients and their families in various care settings.
• Develop individualized social work care plans and coordinate discharge strategies effectively.
• Act as a liaison between hospice staff and inpatient/nursing home teams to ensure seamless care.
• Participate in interdisciplinary team meetings to ensure comprehensive and holistic patient care.
• Conduct pre- and post-bereavement support services for families, fostering healing and resilience.
• Maintain accurate clinical documentation and proficiently input data using relevant software tools.
• Provide weekend coverage as needed to address patient needs and ensure continuity of care.
• Engage in ongoing professional development to enhance skills and knowledge in hospice care.

Job Requirements

• Master of Social Work degree from an accredited institution is required.
• Active LMSW or LCSW licensure in New York State is mandatory.
• Minimum of one year of experience in a healthcare social work setting is essential.
• Proficiency in Microsoft Word and Excel for effective documentation and reporting.
• Ability to work collaboratively within an interdisciplinary team environment.
• Strong organizational, communication, and listening skills are critical for success.
• Experience or internship in hospice or palliative care is preferred but not mandatory.
