Key Responsibilities & Duties

  • Provide psychosocial support to hospice patients and their families in various care settings.
  • Develop individualized social work care plans and coordinate discharge strategies effectively.
  • Act as a liaison between hospice staff and inpatient/nursing home teams to ensure seamless care.
  • Participate in interdisciplinary team meetings to ensure comprehensive and holistic patient care.

Job Requirements

  • Master of Social Work degree from an accredited institution is required.
  • Active LMSW or LCSW licensure in New York State is mandatory.
  • Minimum of one year of experience in a healthcare social work setting is essential.

  • Ability to work collaboratively within an interdisciplinary team environment.
