Job Description

Overview

  • Lead and manage case management operations in a dynamic healthcare setting, ensuring compliance with policies and regulatory standards.
  • Coordinate patient care and discharge planning processes across hospital departments and external agencies for optimal resource utilization.
  • Develop and implement performance improvement strategies to enhance departmental outcomes and operational efficiency.
  • Collaborate with interdisciplinary teams and stakeholders to achieve organizational goals and maintain continuity of care.
  • Oversee staffing, onboarding, and competency development to ensure high-quality case management practices.
  • Analyze data to monitor performance and implement sustainable improvements in case management processes.
  • Adapt quickly to changes and manage multiple priorities effectively in a fast-paced environment.
  • Provide leadership and mentorship to the case management team, fostering a culture of trust and collaboration.

Key Responsibilities & Duties

  • Direct and evaluate departmental operations, including staffing, technology use, and adherence to care coordination processes.
  • Ensure compliance with organizational policies and external regulatory requirements in all case management activities.
  • Collaborate with financial and clinical leaders to achieve productivity and quality objectives.
  • Establish and maintain relationships with key stakeholders to support departmental goals and initiatives.
  • Monitor and analyze operational data to identify areas for improvement and implement effective strategies.
  • Lead the integration of patient care and discharge planning processes across departments and external agencies.
  • Provide guidance and support to staff, fostering professional development and enhancing team performance.
  • Ensure proper documentation and timeliness in care coordination and discharge planning activities.

Job Requirements

  • Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
  • Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) required.
  • Minimum of 3 years of acute care hospital case management experience required; 5 years preferred.
  • Proven leadership experience in case management, including team building and change management.
  • Strong analytical skills with the ability to evaluate data and implement process improvements.
  • Excellent communication and presentation skills, with the ability to engage and motivate teams.
  • Ability to manage multiple priorities and adapt to a fast-paced healthcare environment.
  • Experience in reducing length of stay and improving capacity management in hospital settings.
