Key Responsibilities & Duties
- Direct and evaluate departmental operations, including staffing, technology use, and adherence to care coordination processes.
- Ensure compliance with organizational policies and external regulatory requirements in all case management activities.
- Collaborate with financial and clinical leaders to achieve productivity and quality objectives.
- Establish and maintain relationships with key stakeholders to support departmental goals and initiatives.

Job Requirements
- Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
- Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) required.
- Minimum of 3 years of acute care hospital case management experience required; 5 years preferred.
- Proven leadership experience in case management, including team building and change management.
- Strong analytical skills with the ability to evaluate data and implement process improvements.
- Excellent communication and presentation skills, with the ability to engage and motivate teams.
- Ability to manage multiple priorities and adapt to a fast-paced healthcare environment.
- Experience in reducing length of stay and improving capacity management in hospital settings.
