Job Description

Overview

  • Provide temporary social work services in a healthcare setting, focusing on patient-centered care and support.
  • Conduct biopsychosocial assessments and implement advanced directives and end-of-life planning.
  • Coordinate care plans and facilitate discharge planning for patients.
  • Ensure compliance with documentation standards using electronic medical records (EMR).
  • Collaborate with interdisciplinary teams to provide comprehensive care services.
  • Temporary contract role with potential for extension based on performance and organizational needs.
  • Work on-site during standard business hours, Monday through Friday.

Key Responsibilities & Duties

  • Complete MDS and biopsychosocial assessments for patients.
  • Facilitate care plan meetings and discharge planning processes.
  • Implement advanced directives and end-of-life planning strategies.
  • Maintain accurate and timely documentation in electronic medical records (EMR).
  • Coordinate care and services, ensuring effective person-centered care plans.
  • Monitor ongoing patient care and adjust plans as needed.
  • Collaborate with healthcare teams to address patient needs and improve outcomes.

Job Requirements

  • Master’s Degree in Social Work (MSW) required; LMSW preferred.
  • Minimum of 1 year of experience in social work; long-term care experience preferred.
  • Strong clinical, communication, and organizational skills.
  • Knowledge of community resources and patient-centered care principles.
  • Proficiency in electronic medical records (EMR) systems.
  • Ability to demonstrate judgment in safety and confidentiality issues.
  • Strong customer service focus and ability to work collaboratively.
