Key Responsibilities & Duties

  • Manage medical cases for assigned clients, coordinating care with healthcare teams.
  • Conduct routine health checks and document findings meticulously.
  • Collaborate with physicians and pharmacies to address client health needs.
  • Maintain accurate medication administration records and ensure compliance.
  • Educate clients on health practices, advocating for their wellness goals.
  • Participate actively in treatment planning and case conferences.
  • Provide emergency medical assistance, including first aid and CPR.

Job Requirements

  • Bachelor of Science in Nursing (BSN) and active RN license required.
  • Minimum of 1 year of nursing experience; 3 years preferred.
  • Proficiency in medical case management and inpatient care practices.
  • Experience in maintaining medication records and ensuring compliance.
  • Strong collaboration skills with interdisciplinary teams and external providers.
  • Excellent communication skills for client education and advocacy.
  • Availability to work Wednesday to Saturday, 9 AM to 7 PM.
