Key Responsibilities & Duties
- Manage medical cases for assigned clients, coordinating care with healthcare teams.
- Conduct routine health checks and document findings meticulously.
- Collaborate with physicians and pharmacies to address client health needs.
- Maintain accurate medication administration records and ensure compliance.
- Educate clients on health practices, advocating for their wellness goals.
- Participate actively in treatment planning and case conferences.

Job Requirements
- Bachelor of Science in Nursing (BSN) and active RN license required.
- Minimum of 1 year of nursing experience; 3 years preferred.

- Strong collaboration skills with interdisciplinary teams and external providers.
- Excellent communication skills for client education and advocacy.
- Availability to work Wednesday to Saturday, 9 AM to 7 PM.
