Job Description

Overview

  • Provide comprehensive care coordination services to patients, ensuring optimal healthcare outcomes and efficient resource utilization.
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans.
  • Support patients in navigating healthcare systems and accessing necessary services.
  • Ensure compliance with healthcare regulations and organizational policies.
  • Maintain accurate and timely documentation of patient care activities.
  • Participate in quality improvement initiatives to enhance patient care delivery.
  • Work on-site during designated hours, including occasional weekends.
  • Contribute to a supportive and collaborative healthcare environment.

Key Responsibilities & Duties

  • Assess patient needs and develop personalized care coordination plans.
  • Facilitate communication between patients, families, and healthcare providers.
  • Monitor patient progress and adjust care plans as necessary.
  • Educate patients and families about health conditions and available resources.
  • Coordinate transitions of care and discharge planning activities.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent required.
  • Minimum of 2 years of experience in a healthcare coordination or nursing role.
  • Active Registered Nurse (RN) license in good standing.
  • Strong knowledge of healthcare systems and care coordination practices.
  • Excellent communication and interpersonal skills.
  • Ability to work on-site during designated hours, including occasional weekends.
  • Proficiency in electronic health record (EHR) systems preferred.
