Job Description

Overview

  • Provide comprehensive healthcare services to clients in a women's shelter setting.
  • Collaborate with medical and psychiatric professionals for optimal client care.
  • Work on-site in a supportive and structured environment.
  • Ensure the health and wellbeing of clients through direct medical interventions.
  • Contribute to the development of client health goals and education plans.
  • Participate in treatment team meetings and case conferences.
  • Maintain accurate medical records and medication logs.
  • Provide emergency first aid and CPR assistance as needed.
  • Support a diverse population with varying healthcare needs.

Key Responsibilities & Duties

  • Supervise assigned staff and coordinate client care activities.
  • Link clients to medical and psychosocial resources and referrals.
  • Conduct health and vitals checks for assigned clients.
  • Document client health findings and maintain medication records.
  • Advocate for client health preferences and develop health goals.
  • Coordinate with nursing leadership and team members for client outcomes.
  • Provide health education and emergency medical assistance.
  • Participate in team meetings and ensure adequate medication supply.
  • Perform related duties as assigned to support client care.

Job Requirements

  • Active LPN license required for practice.
  • Associate of Applied Science (AAS) in Nursing or related field.
  • Minimum of 1 year of experience in a healthcare setting.
  • Preferred 2 years of experience providing direct patient care.
  • Proficiency in maintaining medical records and medication logs.
  • Ability to provide health education and emergency medical assistance.
  • Strong communication and collaboration skills.
