Overview
- Lead the case management team in a dynamic healthcare environment, ensuring optimal patient care and resource utilization.
- Coordinate hospital-based case management activities to align with organizational goals and regulatory standards.
- Promote patient-centered care through effective supervision and administrative oversight.
- Serve as a clinical expert and resource for case management practices and policies.
- Monitor quality and compliance of case management documentation and assessments.
- Ensure effective care coordination across a 612-bed academic medical center and freestanding children’s hospital.
- Collaborate with multidisciplinary teams to achieve quality outcomes and patient throughput.
- Support professional development and training for case management staff.
Key Responsibilities & Duties
- Oversee daily operations of the case management program, ensuring alignment with hospital policies and regulations.
- Supervise and monitor the performance of case management professionals and support staff.
- Coordinate activities to promote quality outcomes and efficient patient throughput.
- Act as a subject-matter expert and advisor for case management practices.
- Ensure compliance with applicable regulations and guidelines in case management processes.
- Monitor and improve the quality of interactions, documentation, and assessments by the case management team.
- Collaborate with interdisciplinary teams to optimize resource utilization and patient care.
- Provide leadership and mentorship to enhance team performance and professional growth.
Job Requirements
- Bachelor’s Degree in Nursing or Master’s Degree in Social Work required.
- Current licensure as a Registered Nurse or Licensed Clinical Social Worker required.
- Minimum of 3 years of clinical nursing or hospital social work experience required.
- At least 2 years of supervisory or leadership experience in an acute care setting required.
- Certification in Case Management and/or InterQual preferred.
- Experience with InterQual or IQCI tools preferred.
- Strong knowledge of case management practices and healthcare regulations.
- Excellent leadership, communication, and organizational skills.
