Team Leader Hospice

Overview

Key Responsibilities & Duties

• Provide direct patient care, managing a reduced caseload while ensuring high-quality service delivery.
• Supervise and mentor hospice nursing staff, guiding their professional growth and development.
• Assist in staff orientation and training, ensuring seamless integration into the team.
• Collaborate with management to implement strategic initiatives and operational improvements.
• Conduct field visits to oversee case management and provide support to nursing staff.
• Participate in quality assurance activities to maintain compliance with regulatory standards.
• Facilitate communication between field staff and management, ensuring alignment of goals.

Job Requirements

• Bachelor of Science in Nursing (BSN) from an accredited institution preferred.
• Minimum of six years of hospice nursing experience required; supervisory experience preferred.
• Active New York State RN licensure required; CHPN certification preferred or obtained within one year.
• Proficiency in Microsoft Word, Outlook, and Windows applications necessary.
• Strong ability to supervise and mentor hospice nursing staff effectively.
• Excellent communication and organizational skills to manage field and office responsibilities.
• Commitment to maintaining compliance with hospice and palliative care standards.
• Capability to balance patient care, staff supervision, and professional development tasks.
